'What do I need to include??
Clinical Chart note copies
All relevant pre, and current clinical chart records including intake and registration forms, specific injury/accident questionnaires, diagnostic imaging or other investigative reports (i.e. nerve conduction) surgical/procedural reports, specialist consultation reports, AB forms, medical updates/progress reports, discharge summaries. These should be in MS Word or .pdf format and organized chronologically or in separate folders as the case may be.
Report Request by Third Party
Please include a copy of the written report request and any authorizations.
Signed Service Agreement
Please include a copy of the signed service agreement authorizing Report Medics to prepare your document.

Many clinicians are simply this ... clinical! Our report writing days from University are long since past and some of us were much better at it than others. Unfortunately, if you have motor vehicle accident (MVA) or personal injury clientele as part of your caseload ... it's just a matter of time before you receive a request for a medical-legal report. For some of the more experienced therapists these requests can often be quite numerous. Report Medics is here to take some of these requests and do much of the heavy lifting for you. We will generate a professional report from your patient file records and it returned to you on a timely basis. It will be yours for review and to complete any edits you feel may be warranted. Simply add your fee for any extra time spent and submit it to the requesting agency. It's that simple.
